Piercings ID requirements for minors
The minor must be accompanied by a parent or legal guardian.
Original or certified copy of the minors Birth Certificate must be present.
If the names on the Birth Certificate do not match the adult and minor we will need the legal paperwork showing legal guardianship.
In addition to the parent's photo ID we will need a photo ID for the minor as well.
Acceptable photo IDs for those under 18 are:
License
Passport
State issued ID card
Recent school ID
Ident-a-Kid card
Please remember to bring a valid state/government ID with you, as it is required for all services
Passport, drivers license, and state issued ID are all forms of acceptable identification.
