Cancellation/No Show Fee- $40
If you are more than 10 minutes late to your scheduled appointment time you will be charged a no show fee. If you think you'll be running late to your appointment, please give us a call!

The minor must be accompanied by a parent or legal guardian.
The parent must have a valid state or government issued ID, examples include: License Passport State issued ID card
The minor must have a photo ID, examples include: License Passport State issued ID card Recent school ID Ident-a-Kid card
Original or certified copy of the minors

Original or certified copy of the minors Birth Certificate must be present. If the names on the Birth Certificate do not match the adult and minor we will also need the legal paperwork showing legal guardianship.
